Private Health Fund Rebates

Community Fit offers private health fund rebates as part of our personalised exercise program service.

Each private health fund will have it’s own specific requirements from both the fund member and the service provider to be eligible for rebate, so it is important to find out this information before making any claims. To find out if you are eligible for rebates for your exercise sessions, visit your private health provider’s website for more information.

Generally, to be eligible, most private health funds require members to:

• Be on a specified level of health cover

• Provide a document signed by the member’s GP disclaiming that they are participating in an exercise service for the purposes of managing a diagnosed health condition (this can be found on your health fund provider’s website)

• Provide specified information about the service provider and the service/s being provided.
